Revitalized Med Spa (“we,” “us,” “our”) respects your privacy. This Policy explains what we collect, how we use and share it, and the choices available to Colorado residents under the Colorado Privacy Act (CPA). This Policy applies to:

Our advertising lead forms (e.g., Google Ads Lead Form)
Phone, text/SMS, email, and in-person inquiries about our services
Not for emergencies or health details. Please don’t use the Site or ad lead forms to share medical history or urgent issues. Call 911 for emergencies. Health details are collected in clinic or via designated clinical intake only.
1) Personal information we collect

Sensitive data. We do not intentionally collect sensitive data via the Site or ad lead forms (e.g., health condition, biometric data, or data from a known child). If sensitive info is ever needed, we’ll obtain opt-in consent and use appropriate intake methods.

3) Cookies, analytics, and ads

We share information only as needed to run our business, including with:
Service providers (website host, scheduling & messaging tools, analytics/ad platforms) under contracts limiting their use of data

We do not sell personal data.
5) Retention
We keep personal information only as long as needed for the purposes above or as required by law (e.g., records related to clinical care, tax, or audit). If you ask us to delete your info, we’ll do so unless we must keep it for legal or legitimate business reasons.
6) Security
We use reasonable administrative, technical, and physical safeguards designed to protect personal information. No system is 100% secure; please do not submit sensitive medical details through general web forms or email.
7) Your Colorado privacy rights (CPA)
Colorado residents may exercise the following rights (subject to verification):

(a) targeted advertising, (b) the sale of personal data (we don’t sell), and (c) profiling that produces legal or similarly significant effects.Access your personal data we process.
Correction of inaccuracies.
Deletion of personal data.
Data portability: receive certain data in a portable, usable format (up to twice per year).
How to exercise your rights
Email info@getrevitalizedmedspa.com with the subject “Colorado Privacy Request” and tell us which right(s) you want to exercise. We’ll verify your identity and respond within 45 days (we may extend once by up to 45 days when reasonably necessary and will let you know why). If we can’t act on your request, we’ll explain why and how to appeal.
Opting out of targeted advertising
Use our site controls (cookie/settings link, where available) or email us.
We recognize approved universal opt-out signals (e.g., Global Privacy Control) from Colorado residents’ browsers and treat them as an opt-out of targeted advertising (and sale if applicable).
You may still see ads, but they won’t be tailored using cross-site tracking.
Appeals process
If your request is denied, email info@getrevitalizedmedspa.com with the subject “Colorado Privacy Appeal.” We’ll provide a written decision within 45 days (we may extend by up to 60 days when reasonably necessary and will notify you of any delay). If you remain concerned, you can contact the Colorado Attorney General: coag.gov (Office of the Attorney General, Colorado).
8) Children’s privacy
Our Site/services aren’t directed to children under 13. We don’t knowingly collect personal information from children under 13. Clients under 18 require parent/guardian involvement for medical services.
9) HIPAA notice (website vs. clinic)
Some med-spa services may involve health records governed by healthcare privacy laws and stored in clinical systems. The website and ad lead forms are not intended for PHI. Share medical information only through our designated clinical intake or in-person.
10) Financial incentives & promotions
We may offer promotions or discounts. Participation is voluntary and you can withdraw at any time. We don’t condition service on waiving your privacy rights.

12) Changes to this Policy
